I set up all headings in styles 1,2,3. Do I have to actually create a TOC, or is that something that gets generated automatically?

Hello rickl, welcome to the forum!
What software do you work with? I use Sigil. You can have it created there.
There are two types of Table of content (TOC): the HTML TOC and the TOC.NCX file (which is used for navigation on Kindles).
Here's how to compile them in Sigil:
For the TOC.NCX: Tools > Table of Contents > Generate Table of Contents
For the HTML TOC: Tools > Table of Contents > Create HTML Table of Contents@maggie

I set up all headings in styles 1,2,3. Do I have to actually create a TOC, or is that something that gets generated automatically? I encountered many problems with heading 3 at Lulu. Now I just avoid them altogether. I give them a different style name like "bold three". Avoid numbers in styles. This will also trigger errors in Sigil. Update TOC (generate toc) after every change otherwise you will get an "invalid" notification when uploading.
